Adult 5AT-HOME RESIDENTIAL HABILITATION (Res-Hab)

At-Home Res-Hab offers services to individuals who live at home with their family. The program offers choices and increased independence, encourages productivity and shared community involvement. Res-Hab staff assist in personal care, meal preparation, money management and household skills.

Programs include:

Community Access Training Grant
This grant provides funding for recreational activities for home bound children and adults with disabilities.

Hourly Medicaid Waiver Respite
Provides ongoing and short term hourly respite and supervision services.

Educational Toy Closet
A program that allows families to borrow or acquire adapted toys or equipment that suit the functional and recreational needs of both children and adults with disabilities.

Family Support Services/At-Home Overnight Respite
Overnight respite provides short- term 24 hour a day support and supervision to an individual whose primary caregiver is temporarily absent. This service may be utilized in either emergency or recreational situations. For the greatest level of comfort, the family chooses their respite provider.
