What is Service Coordination?
Service Coordination is the referring and linking of families and/or individuals with community services. Service coordinators accumulate knowledge of these resources, sharing them with individuals and families at their time of need.
The purpose of the program is to provide the guidance and counseling necessary, throughout all areas of their lives, assuring individuals are involved and supported in their community with the goal of achieving independence.
What are the eligibility requirements?
Social Work Services are offered to children or adults with disabilities. Services are also available to parents of children with disabilities to assist in their emotional needs.
What is Medicaid Waiver?
As describes by NYS: “The Care At Home waiver program allows Medicaid to pay for some services not provided through "regular" Medicaid, such as case management, respite and home adaptations. Availability of these services can make home care an option for children and their families. Although Medicaid funding obtained applies only to the child, it can be used to defray the costs of caring for the child at home”.

Monday through Friday 8am – 4 pm with 24 hour on call service.
